At CFM, we’re always working to do better - for our patients, our team, and our community. That’s why we’re excited to be making the switch to Epic, a leading electronic medical record system used by top healthcare systems across the country.

Starting July 9, 2025 you will need to:

  • Bring a photo ID to your first appointment after July 9.
  • Bring Insurance Card(s) to your first appointment after July 9.
  • Allow extra time for appointments.

Note: Since CFM is a service of Hannibal Regional, your MyChart account will be accessed under the Hannibal Regional organization. You will have full access to your CFM provider and records through this shared platform.

Activate Your Account

Activate Account With A Code

When you visit a Complete Family Provider provider but do not have a MyChart account, you will receive an activation code in the After Visit Summary given to you when your visit is complete.

Click Sign Up Now on the Hannibal Regional MyChart website. Note: Since CFM is a service of Hannibal Regional, your MyChart account will be accessed under the Hannibal Regional organization. You will have full access to your CFM provider and records through this shared platform.

Enter your activation code and date of birth in the boxes provided on the page.

Click the Next button, and you will be directed to create a user name and password.

You will also be asked to verify whether you wish to enable email or text notifications.

Set Communication Preferences

Communication Preferences

Manage which MyChart alerts you wish to receive – and how to receive them. Receive notices of appointments, test results, billing statements and more. Choose whether you want to be notified by email, text message, push notifications or all options.

Review Contact Information

To review your contact information, scroll the Menu to Account Settings and click on .

Review the Contact Information section to add or edit any details including your mobile phone number, your email address and your home phone and mailing address. By selecting the star next to a specific phone number listing, you can set it as your preferred number for communications.

This area is also helpful if plan to travel for an extended period of time. Enter a temporary address along with your start and end dates to ensure you are receiving timely information while you’re away.

Set Up Communications

Please follow the instructions above to access the Communication Preferences page. Once you have confirmed that your contact information looks accurate, you’re ready to select how you prefer to receive notifications.

Under Details, you’ll see several categories to choose from:

  • Appointments
  • Messages
  • Health
  • Billing
  • Questionnaires
  • Account Management
  • Telehealth
  • To Do

Click the down arrow next to the category name to toggle your notifications on or off based on your preference: email, text or push notification. Green toggle buttons mean you are opted in, and gray toggle buttons mean you are opted out. Locked icons (like those designated as mail) cannot be changed.

You may also modify notification settings for individual topics within a category grouping by clicking advanced settings within each category.

Once you have set your preferences, click Save Changes at the bottom of the screen.

Family Access

How To Request Proxy Access

You can use your MyChart account to request access to medical records of family members – or invite them to access yours. This is called proxy access, and some limitations may apply. Please Note: Individuals with proxy access will be able to view the same personal health information that you are able to access within your own individual account. We recommend that you periodically review who has proxy access to your account.

Ages 0-11: Scroll the Menu to the Communication section and click on Request Family Access. Click on the link “Submit request to access a minor’s record.”

Ages 12 - 17: Access for a child age 12-17 must be requested directly through your child's physician's office. A form can be completed in person or by phone with the patient and parent/legal guardian. This request cannot be submitted through MyChart.

Ages 18+: You can receive permission to access the Hannibal Regional MyChart accounts of adult family members, or invite them to access your account. From your home page, scroll the Menu to the Communication section and click on Request Family Access. Then click the “Friends and Family Access” link. From there, click “Invite Someone” and follow the prompts to complete the request.

Note: If an adult family member is incapacitated or unable to communicate, you may request access to their account by submitting a form and relevant legal documents (such as power of attorney) to your loved one’s provider or the hospital. A printable copy of the form for adult family members can be downloaded from a link on the Request Family Access page. The paperwork will need to be completed with the patient's primary care provider to properly document that the patient is incapacitated or unable to communicate.

How to Revoke Access

From your home page, scroll the Menu to the Communication section and click on Request Family Access. Then click the “Friends and Family Access” link. From there, you can revoke access. If you are a child age 12-17 and no longer wish for a parent or guardian to have proxy access, please contact your local Hannibal Regional facility to complete the MyChart Proxy Revocation Form.

Limitations

The following limitations apply for MyChart access: 

  • If your child is age 0-11: You will be granted full access to your child’s MyChart account. 
  • If your child is age 12-17: Due to federal and state laws, when a child reaches the age of 12, access to their MyChart account will become limited to parents/legal guardians. A MyChart Child 12-17 Authorization Form must be requested directly through your child's physician's office.  Limited access will only include access to allergies and immunizations on file.
  • If legal decisions, rulings, or judgments related to parental rights have been made that may impact MyChart access, additional information may be required.
  • If we are unable to validate your relationship to the child (within either age range), you may be asked to provide a copy of their birth certificate.
  • Once your child turns 18: Proxy access is discontinued. Review the information above for help accessing an adult account through MyChart. 

Note: these age range limitations do not affect any legal right you have to access your child’s record by other means. To request a paper copy of your child’s record, please contact the Complete Family Medicine facility where care was provided to obtain record copies.

Link Your Accounts

How To Link Accounts

You can use Hannibal Regional MyChart to view your health information from other Epic organizations where you have records by linking your accounts.

Log into your MyChart account and scroll the Menu on your home page to the Sharing section. Click on Link My Accounts and search for the organization where your records are stored. You will need your log-in information for that organization's account to access and link your records.

Share Your Information

How To Share

With a Hannibal Regional MyChart account, you can share your own information with providers in other health systems, employers or anyone you choose – as long as they have access to shareeverywhere.com.

Log into your MyChart account and scroll the Menu on your home page to the Sharing section. Click on Share Everywhere. Enter the name of the person with whom you want to share your record and click the button to “Request share code.” You’ll receive a one-time code to share with that person, who will also need your birthdate to access your record. The one-time code is valid for only 60 minutes. Your record can be accessed only one time. As soon as the person with whom you share your record logs out, they will not be able to access your record again unless you repeat the process and provide another code.
